      3+1 PROFESSIONAL TRAINING MODEL INTERNSHIP/PROFESSIONAL TRAINING FORMS

      The following forms are required for those who will conduct their professional training/internship in Giresun under the 3+1 Professional Training Model. The necessary documents can be accessed through the links below. Students who will do their professional training/internship outside of Giresun must also prepare the Professional Training/Internship Acceptance Form and Internship Agreement documents, and if there are any other forms required by their institutions, they must prepare these forms, have them signed by the institutions, and submit them to the program internship supervisor.

      Additional Documents Required by Institutions Affiliated with Giresun Provincial Health Directorate for 3+1 Term Internship and Professional Training Courses
      Required Documents for Internship Application (Please read carefully)
      • 3- Student Confidentiality Agreement (1 Copy) PDF To be filled and signed by the student and submitted to the relevant healthcare facility. This form should be submitted to all institutions except Giresun Training and Research Hospital.
      • 3- Personnel Confidentiality Agreement (1 Copy) PDF To be filled and signed by students who will do their internship at Giresun Training and Research Hospital.
      • 4- Student Periodic Medical Examination Form (2 Copies) PDF
      • 5- Health Screening Form (2 Copies) PDF Current test results must be approved and submitted to the relevant healthcare institution.
      • 6- Occupational Health and Safety Certificate Students will need to present a transcript for this document, which can be obtained from SHMYO Student Affairs Office.
      • 7- Insured Employment Entry Document To be obtained from the e-government portal's 4a employment entry-exit document section a few days before the start of training/internship.
      • 8- Student ID Card with Photo This document will be obtained from SHMYO Student Affairs Office.
      • 9- Ziraat Bank Account Form To be obtained by the student from the bank branch or through online banking services.
      • 10- Identity Card Copy (2 Copies)
